If you or someone close to you needs extra support to continue living at home safely and independently, a NASC assessment is the first step toward accessing funded services. HealthCare NZ works alongside NASC agencies nationwide to ensure individuals and their whānau receive the right care, at the right time, in the comfort of their own home.

What is NASC?

NASC stands for Needs Assessment and Service Coordination: a government-funded service designed to identify your support needs and connect you with appropriate services.

A NASC assessor can meet with you (and your whānau, if you choose) to understand your daily routines, health challenges, and living environment. Based on this assessment, you may be eligible for services such as:

  • Personal care and hygiene support
  • Household assistance and meal preparation
  • Respite and recovery care
  • Long-term condition management
  • Rehabilitation and specialist services

Yes. After your assessment, you can ask to be referred to HealthCare NZ. We’ll work with your NASC team to ensure a smooth transition into care.
